SMS/Messaging Policy
Serene Resolutions uses secure messaging tools, including Quo, to communicate with patients, caregivers, facilities, and care partners as part of providing healthcare and care coordination services.
How We Use Text and Secure Messages
We may send messages related to:
- Care coordination and service updates
- Scheduling or appointment-related communication
- Care plan information and follow-ups
- Requests for information necessary to support services
- Administrative or operational updates related to your care
We do not use text messaging for marketing or promotional purposes unless you have explicitly opted in.
Messaging Platforms and Third-Party Providers
To deliver these messages, we work with trusted third-party service providers (such as secure messaging platforms, telecommunications providers, and technology vendors). These providers may process limited personal information solely to support message delivery and system functionality.
We do not sell, rent, or share your mobile number or SMS opt-in data for advertising or unrelated purposes.
Privacy and Security
Messaging may contain personal or health-related information and is handled in accordance with applicable privacy laws, including HIPAA. Serene Resolutions takes reasonable safeguards to protect the confidentiality and security of information communicated through approved messaging platforms.
Opt-In and Opt-Out
You may choose to opt out of SMS or secure messaging at any time by following the opt-out instructions included in messages or by contacting us directly. Opting out of messaging will not affect your ability to receive services, but may limit our ability to communicate efficiently.

HIPAA Notice of Privacy Practices

This Notice applies to all protected health information maintained by Serene Resolutions LLC and describes how medical information about you may be used.
1. Purpose
This Notice of Privacy Practices (“Notice”) describes how medical information about you may be used and disclosed by the Company and how you can access this information, in accordance with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), as amended, and applicable Florida law.
2. Scope
This Notice applies to all patients receiving healthcare services from the Company, including services provided in assisted living or residential care settings.
3. Our Legal Duties
The Company is required by law to maintain the privacy and security of your protected health information (“PHI”), to provide you with this Notice, and to follow the terms of this Notice currently in effect.
4. Permitted Uses and Disclosures of PHI
The Company may use and disclose your PHI for treatment, payment, and healthcare operations. This includes coordinating care with healthcare providers and assisted living staff, billing health insurance plans, Medicare, Medicaid, and conducting quality and compliance activities.
5. Special Protections for Certain Information
Some types of health information, such as information related to behavioral health services, substance use, or HIV status, may be subject to additional protections under federal or Florida law. When applicable, Serene Resolutions LLC will comply with those additional legal requirements.
6. Uses and Disclosures Required or Permitted by Law
The Company may disclose PHI as required or permitted by federal or Florida law, including for public health activities, health oversight, and to prevent a serious threat to health or safety.
7. Uses and Disclosures Requiring Authorization
Uses and disclosures of PHI not described in this Notice will be made only with your written authorization, which you may revoke at any time in writing.
8. Individual Rights
You have the right to access and obtain copies of your PHI, request amendments, request confidential communications, request restrictions, receive an accounting of disclosures, and obtain a paper copy of this Notice.
9. Complaints
If you believe your privacy rights have been violated, you may file a complaint with the Company or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint. 10. Changes to This Notice
The Company reserves the right to change this Notice at any time. Any changes will apply to all PHI maintained by the Company. The revised Notice will be available upon request and posted where services are provided.
